Paraneoplastic Cerebellar Degeneration with Anti-Yo Antibody Manifested by Central Nystagmus and Abnormal Head Impulse Test

Abstract
Anti-Yo antibody is the most frequently detected autoantibody in paraneoplastic cerebellar degeneration which manifests progressive gait and limb ataxia, vertigo and nystagmus. Here we report the case of a 42-year-old woman who was diagnosed with paraneoplastic cerebellar degeneration with anti-Yo antibody associated with breast cancer. The patient showed abnormal head-impulse tests with perverted catch-up saccades compatible with the cerebellar dysfunction along with spontaneous downbeat, gaze-evoked and rebound nystagmus. Even after active treatments such as surgical tumor resection, chemotherapy, radiotherapy, and immunotherapy, the symptoms persist for more than one year.
